Additional COVID-19 Safety Regulations will be in effect for all home games, practices, and other events. Neighboring communities will have their own policies in place for their home events that must be followed. All athletes and volunteers must complete the PCSD COVID-19 Waiver to practice at PCSD facilities.

As a condition of using the softball fields located at Portage High School, ALL PARTICIPANTS (including players, coaches, umpires, visiting coaches, and visiting players) MUST complete this waiver.
This waiver will not be submitted digitally. It must be printed, signed, and turned into your head coach. You will NOT be allowed on any PCSD fields, dugouts, batting cages, etc. without this being turned in to your head coach.
For opposing teams, all players and coaches must complete this waiver and turn them in to the head coach of the PYS team you are playing.
For umpires, turn this in to the PYS head coach of your first game. You will not need to submit one for each game.
